Everything comes down to one thing: what you plan to treat. The sanatoriums in Mariánské Lázně have different profiles. Some focus on kidneys and metabolism, others on the musculoskeletal system, respiratory system, and skin.
Since July 24, 2021, the resort has been listed as a UNESCO World Heritage Site as part of the "Great Spa Towns of Europe" (as of 09/2026, whc.unesco.org). The mineral springs in the town are open to everyone and free of charge. You pay only for your room and the course of treatments.
We've gathered the sanatoriums in Mariánské Lázně that offer treatment and organized them by profile, category, and distance to the colonnade. Some hotels accept children, others are adults only.

How to choose a sanatorium in Mariánské Lázně
Look at the indications first, then at the stars. Nove Lazne, Esplanade, and Rubezahl-Marienbad all have five stars, but they treat different things. The category tells you about the rooms and service, not the strength of the treatment facilities.

- Check where the treatment center is located: in your building or in a separate town facility you'll need to walk to.
- Check the distance to the colonnade. The difference between hotels can be up to half an hour on foot.

1. Nove Lazne (5*)

2. Esplanade spa and golf resort (5*)

10. Royal (4*)

Every room and the terrace offer a panoramic view of the resort. The hotel features a wellness and spa complex, an indoor pool, a Turkish bath, a sports equipment rental point, and a small equestrian club. The restaurant serves Czech cuisine and homemade desserts from the head chef.

11. Krivan (4*)

12. Spa Resort Hvezda (4*)

The center is housed in a historic building with a terrace, a few minutes' walk from the Cross Spring Colonnade. They work with classical methods here, so comprehensive treatment courses produce predictable results. Mineral water is piped in for carbon dioxide baths, and water from the Balbin spring is used for drinking. Guests have access to the largest pool at the resort and Roman baths.

13. Grandhotel Nabokov (4*)
The sanatorium combines seven historic buildings. The medical department has a strong diagnostic base and a wide selection of programs, plus a pool and two wellness centers. Water is piped in from the Lesni spring. Guests get free access to the pool, saunas, and steam rooms, and breakfast is served as a buffet. The sanatorium is a five-minute walk from the center, almost at the edge of the forest. The rooms are bright, the furniture is natural wood, and mineral waters are piped in for drinking and treatments, including the Balbin spring. The doctors have worked at this sanatorium for over thirty years and specialize in urology and the musculoskeletal system. Children are accepted from age six, so families come here too.

16. Butterfly (4*)

The hotel sits on a rise directly opposite a small park that's convenient for walks. The Edward and Augustin springs are a few minutes' walk away, and the treatment department operates in the building itself. It has its own distinctive feature too: sulfate baths using piped-in water from the Ferdinand spring. They are prescribed for musculoskeletal conditions and skin problems. Peat mud is applied directly to the body rather than in packs.

22. Imperial (4*)
Unlike many sanatoriums at the resort, Imperial does not occupy a historic building. It is a modern complex of the Hvezda chain, connected to neighboring buildings by covered walkways, so the list of services is broader than usual. The hotel is connected to the Ambrož, Balbín, and Lesní springs. Thanks to the Mariin spring with a very high carbon dioxide concentration, dry gas baths are offered here. Profile: gastrointestinal tract, kidneys, and post-surgery recovery. Dietary meals are available in several types.

26. Devin (3*)

27. Villa Walir (3*)
The villa occupies the quietest part of the resort. The Ferdinand Spring is a five-minute walk away, and the Rudolf Spring pavilion is ten minutes away (according to the map). Medical and wellness treatments are provided in the building itself. Many people come here for a consultation with a doctor who has worked in the sanatoriums of Marianske Lazne and Karlovy Vary for more than thirty years. Because of its compact size, the approach to treatment here is individualized, and the physiotherapy equipment allows for spinal correction work.

28. Schlosshotel Marienbad (3*)
The hotel stands not far from the main city attractions and occupies a bright two-story building with its own balneological center. Mineral water is not piped to the hotel, and the springs are located far away. On the plus side, buses stop right at the hotel, and you can reach the colonnade in five minutes. Treatment is standard, with indications including heart conditions, diabetes, and the musculoskeletal system. By arrangement with the city spa center, guests have free access to the sauna, jacuzzi, and city pool.
- Who it's for: those who want a quiet area and a free city pool.

30. Svoboda (3*)
The sanatorium stands amid greenery, in the center of a forested area. The "Josef" spring, an analogue of the "Lesni" spring but with a higher salt content, is 200 meters away (according to the map). The common areas and rooms were recently renovated, so they look fresh. Treatment follows a classic methodology with maximum use of natural factors, and the testing and diagnostic system is equipped with modern equipment. In terms of finish quality, the building is on par with neighboring 4-star hotels.
- Who it's for: those who want quiet in the forest and modern rooms.

About the resort
The town got its new name and new look in the 19th century. That was when the sanatoriums were built, their neo-Gothic architecture blending into the landscape that locals call an ocean of greenery. The resort sits at elevation and is surrounded by forests, so the air here is different from the valley. In terms of visitor numbers, Marianske Lazne is second only to Karlovy Vary.
There are many springs: about 40 cold springs flow right within the town limits, and about 100 more are scattered around the surrounding area (as of 09/2026, Ensana). These are complemented by therapeutic mud and natural carbon dioxide, and the treatment rooms are equipped with modern medical equipment.
The drinking cure takes place at the colonnade: you fill your cup at the drinking fountains and sip small portions between treatments. In summer, the West Bohemian Symphony Orchestra plays at the colonnade, and the concerts are free (as of 09/2026, resort program). Above the town, a funicular runs to the Boheminium miniature park: the lower station is behind the Nove Lazne hotel, and the car runs every fifteen minutes. If you need a break between treatments, the ride up takes very little time.
What conditions are treated

The respiratory system benefits from mineral baths, inhalations, and drinking cures. Gynecological issues are treated with baths and mineral water irrigations, as well as mud wraps. A separate focus area: the stomach, pancreas, liver, and gallbladder. Drinking from the local springs also has a positive effect on metabolism.

Wellness programs

Types of springs and classification of the water's therapeutic properties

Based on the research, a profile was compiled for the six main springs of the resort. Their water differs in composition, which is why each is prescribed for different diagnoses.
The Křížový (Cross) Spring is named after the wooden cross that once stood beside it. Its water is rich in sulfate and iron. When first used, it was called Glauber's water. This water is drunk for gastrointestinal conditions and helps normalize metabolism. Ferdinand is similar in composition, but has higher mineralization. This spring has subtypes: Ferdinand I is used for drinking, while the others are used for mineral baths. The bottled water "Excelsior" sold in stores is sourced from the Ferdinand IV spring.
Next to the Ferdinand Colonnade is the Rudolf Spring, named after Crown Prince Rudolf, son of Emperor Franz Joseph I. The water is lightly mineralized, with anti-inflammatory properties, higher acidity, and a diuretic effect. It is prescribed for nephritis, cystitis, and other urinary tract conditions. The calcium and magnesium in its composition provide a therapeutic effect for osteoporosis.
The Forest Spring in the northern part of the resort is distinguished by its alkaline composition. Its water is used for hyperacid gastritis and other gastrointestinal conditions, and inhalations with it help the throat and bronchi. Drinking this water helps dissolve acid-based kidney stones.
Ambrož bears the name of the abbot of the Teplá Monastery. It is an acidic, iron-rich water with low mineralization, drunk for anemia and conditions of the urogenital system. The colonnade with drinking springs stands in the center, next to the Singing Fountain. In high season, it plays every odd hour from seven in the morning to nine in the evening. The last show starts at ten in the evening, and the nine and ten o'clock shows include lighting (as of 09/2026, official resort website).
